I. INCIDENT BRIEF
Marine Investigations: On January 11, 2026, at approximately 0150, the 65-foot, 77 gross ton, Commercial Fishing Vessel TEXAS LADY (ON: 624106), with 03 persons and 02 dogs onboard, experienced a grounding on Agate Beach near Port Orford, Oregon. Coast Guard and Curry County emergency services assisted rescuing all persons and dogs onboard. Vessel was hard aground. Owner of the vessel conducted salvage operations with insurance. Owner estimated property damage/vessel worth from insurance to be approximately $575,000. CG-INV Note 004-2025 - NO NTSB or CIC required notification / Routine MISLE Documentation. Alcohol and DOT Drug Testing completed. Initially, no reported injuries. On January 12, 2026, The Operator and 01 Crewmember went to the hospital on their own and was reported that the 01 crewmember sustained injuries that required medical treatment beyond first aid. As a result of its investigation, the Coast Guard has determined that the initiating event for this casualty was the failure of anchor resulting in the grounding of TEXAS LADY. The causal factors that contributed to this casualty include: (1) Failure to maintain a proper anchor watch. The TEXAS LADY was a U.S. Flagged vessel engaged in commercial service. IAW CG-INV Policy Letter 02-22, a Standard Investigation activity was completed. Incident Management Division: Salvage operations for the CFV Texas Lady commenced on 15 January 2026 and were conducted by Global Diving and Salvage under favorable environmental conditions and coordinated oversight. On 15 January 2026, the CG Sector Columbia River Federal On-Scene Coordinator (FOSCR) conducted an initial assessment of operational and environmental risk and determined that on-scene scientific presence could be safely delegated to the State On-Scene Coordinator. This determination was supported by close virtual coordination with Oregon State Parks and a tribal liaison, both of whom maintained continuous scientific awareness throughout the operation. In advance of dismantling activities, the FOSCR obtained Resources at Risk (RAR) information from the NOAA Scientific Support Coordinator. Based on seasonal conditions, the SSC assessed that even in a worst-case scenario involving release of the remaining approximately 150 gallons of hydraulic fluid and red-dyed diesel, environmental impacts would be negligible, due to high wave energy, significant natural dispersion, and the absence of sensitive or critical species in the area during this time of year. During the same six-hour low-tide window on 15 January, salvage crews successfully removed the vessel’s mast, rigging, and superstructure. Removed material was transported up the beach, dismantled, and staged for disposal via semi-trucks. Operations were completed without incident and with no observed pollution outside containment. On 16 January 2026, work continued under low winds and calm surf conditions, allowing for controlled dismantling of the remaining hull. At the start of the day, an estimated 35–50 percent of the vessel remained, with approximately 1,500 gallons of red-dyed diesel and hydraulic fluid recovered to date. Salvage proceeded using a top-down dismantling approach, with sausage boom, absorbent pads, and pumps deployed methodically to ensure all pollutants remained constrained to the bilge. Early on 16 January, the excavator shear was damaged and subsequently replaced. During this pause in mechanical dismantling, crews utilized the additional equipment hours to conduct targeted residual oil recovery, focusing on absorbent pad placement and cleanup of exposed compartments. Following replacement of the shear, dismantling operations resumed as planned. Oregon State Park Rangers remained on scene throughout operations and reported positively on the effectiveness of environmental protection measures. On 17 January 2026 at 1600L, salvage crews completed full removal of the CFV Texas Lady. Final site restoration actions—including beach regrading and gravel replacement on the access road and parking lot—were completed as part of the operation. All work was completed to the satisfaction of the Oregon Department of Environmental Quality (DEQ), Oregon State Parks, and the CG Sector Columbia River Federal On-Scene Coordinator. At no point during the operation was pollution observed outside established containment. This outcome was attributed to calm weather conditions, disciplined execution of a top-down salvage methodology, effective interagency scientific coordination, and the professional performance of salvage crews. All operational objectives have been met. Salvage operations are closed.
