I. INCIDENT BRIEF
|

On 06 May 2020 at 1551, Sector Boston received a R21 VHF/FM Channel 16 call from 41' F/V HEATHER RENEE DIW in POSN 42-48.01N 069-19.183W, approx. 55NM E of Gloucester. Vessel reported a loss of propulsion due to issues with transmission gear and was subsequently taken in tow by CGC KEY LARGO. Vessel arrived safely into port in Provincetown and conducted damage assessment and repairs.
As a result of its investigation, the Coast Guard has determined the initiating event for this casualty to be a vessel equipment failure involving the transmission gear. The gear failure resulted in a loss of propulsion on the vessel. The transmission failure was attributed to possible gear defects or installation issues. The gear was replaced and tested with no further issues detected.
